Increasingly, attention and resources are being focused on addressing social determinants of health (SDOH) as awareness grows that SDOH, or the conditions in which people are born, grow, live, work, and age, are a primary driver of health outcomes. However, collection and exchange of SDOH data is lagging, hampering efforts to address SDOH.
Leavitt Partners and CyncHealth, organizations that carry deep expertise in federal digital health policy and SDOH data collection workflows, respectively, see two significant, interrelated barriers to the collection and exchange of SDOH data. First, a lack of stakeholder understanding of key federal policies affecting the collection and exchange of such data. Second, a lack of understanding of core workflows of SDOH data collection and exchange.
Today, in order to address those barriers, Leavitt Partners and CyncHealth are releasing the Playbook for Social Determinants of Health (SDOH) Referral Management and Data Exchange (“the Playbook”). The Playbook is comprised of two main sections:
- Overviews of significant federal policies that affect the collection and exchange of SDOH data, including HIPAA, as well as an exploration of consent and identity management workflows.
- Overviews of three key workflows and activities within SDOH data collection and exchange: SDOH screenings, referrals to social services, and planning for financial sustainability of SDOH workflows and processes.”
